Creatine does not cause fat gain. The weight increase people notice in the first week or two of supplementing is water drawn into the muscle cell, not fat stored under the skin, and research consistently shows no increase in fat mass in people taking creatine. The "bulky" look people worry about comes from muscle gained through years of training, not from a 5g scoop of powder.
No. This is one of the most directly tested questions in sports nutrition, and the answer has been consistent across decades of research.
A 2021 review in the Journal of the International Society of Sports Nutrition, authored by eleven researchers who have collectively published more than 200 peer-reviewed papers on creatine, examined twelve of the most persistent myths about the supplement. On body composition, their conclusion was unambiguous: creatine supplementation does not increase fat mass.
If anything, the effect trends in the opposite direction. A 2023 meta-analysis published in Nutrients pooled twelve studies involving 266 adults under 50 and found that those who combined creatine with resistance training experienced a small but statistically significant reduction in body fat percentage of 1.19%, with no change in absolute fat mass. In other words: fat mass stayed the same while lean mass increased, so fat made up a smaller share of total body weight.
The pattern holds in older adults too. A separate meta-analysis in the Journal of Functional Morphology and Kinesiology found that adults aged 50 and over who supplemented with creatine during resistance training lost 0.55% more body fat than those training on placebo alone.
Because creatine is osmotically active, meaning it pulls water along with it when it moves into a cell. As your muscles absorb creatine and store it as phosphocreatine (the compound your body uses to rapidly regenerate energy during short, hard efforts), a small amount of water follows it into the muscle tissue.
That water has weight. It is not fat, and it is not permanent in the way fat is.
The International Society of Sports Nutrition's position stand on creatine, the most comprehensive evidence review available, covering more than 500 studies, notes that an increase of roughly 1–2kg in body mass is common in the first week when a traditional loading protocol is used.
That figure is worth putting in context. One to two kilograms of body fat takes months of sustained energy surplus to accumulate. One to two kilograms appearing in five days is a fluid shift, not fat storage. It is the same reason your weight can swing a kilogram after a salty meal or a long-haul flight.
Into the muscle, not under the skin, and the evidence for how much water shifts at all is weaker than most people assume.
The 2021 JISSN review found the case for creatine-driven water retention to be surprisingly shaky. Some short-term studies show an increase in total body water, primarily inside the muscle cell (intracellular). Several others show no change in total body water at all, and no shift in the balance between fluid inside and outside the cells.
This distinction matters for how you look. Water held inside a muscle cell makes the muscle appear slightly fuller and firmer. Water held in the layer between skin and muscle (subcutaneous) is what creates a soft, puffy appearance, and that is not where creatine sends it.

Building visible muscle is slow, effortful work. It requires progressive resistance training, adequate protein, and enough total energy, sustained over months and years. Creatine's role in that process is indirect: the ISSN identifies creatine monohydrate as the most effective nutritional supplement available for increasing high-intensity exercise capacity and lean body mass during training. It helps you do a little more work in the gym. The work is still what builds the muscle.
Put plainly: creatine does not add size. It helps you train hard enough to earn it, if that's your goal. If it isn't your goal, taking creatine will not accidentally produce it.
The mechanism is identical, but the practical outcome differs in one respect. Women generally carry lower baseline creatine stores and gain proportionally less total body mass when supplementing, largely because absolute muscle mass is lower to begin with.
The dose does not need to change. Around 3–5g daily is effective regardless of sex, and the same body composition findings apply, no increase in fat mass, and a small favourable shift in body fat percentage when paired with resistance training.
When someone genuinely gains fat after starting creatine, the cause is almost always something that changed alongside it.

Skip the loading phase.
Loading, typically 20g daily split across four doses for five to seven days, saturates the muscle faster, but it is optional. The evidence is clear that lower daily dosing reaches the same muscle saturation, just over a longer window. The 2021 JISSN review concluded that 3–5g per day (or 0.1g per kg of body mass) is effective, and that a loading phase is not required.
